We are currently recruiting for an experienced Intensive Support Worker to join our client’s Complex Lives service in Belfast.
This is an opportunity to work intensively with individuals experiencing multiple and complex challenges, which may include homelessness, problematic substance use, mental ill health, offending behaviour, poor physical health and trauma.
As an Intensive Support Worker, you will act as a trusted key worker, coordinating wraparound support and helping individuals engage with services, maintain accommodation, improve stability and progress towards their personal goals.

About the Role
Working as part of a multidisciplinary team, you will use a holistic, housing-led, low-threshold and harm-reduction approach to support individuals with complex needs.
You will manage a caseload of approximately 7–10 individuals with highly complex and entrenched needs, providing intensive case management and coordinating support across multiple services.
The role requires strong relationship-building skills and the ability to work effectively with individuals who may experience periods of crisis, disengagement or challenging behaviour.
You will work closely with multidisciplinary partners and community, voluntary and statutory organisations to develop and deliver responsive wraparound support plans.
Key Responsibilities
Your responsibilities will include:
• Managing a caseload of approximately 7–10 individuals with multiple and complex needs
• Acting as a trusted key worker and building consistent, professional relationships with service users
• Providing ongoing case management, including triage, initial support, detailed assessments and care planning
• Developing, implementing and regularly reviewing individualised support plans
• Supporting individuals to engage with appropriate health, housing and community services
• Helping service users stabilise and successfully settle into accommodation
• Supporting individuals to connect with their local communities and make positive lifestyle choices
• Working collaboratively with multidisciplinary partners and external agencies to coordinate wraparound support
• Organising regular key-working sessions according to individual need
• Supporting service users with reflection, action planning and overcoming potential crisis or trigger points
• Monitoring progress and updating support plans to demonstrate outcomes and achievements
• Identifying and addressing barriers that may prevent individuals from progressing
• Developing effective working relationships with community, voluntary and statutory services
• Organising and contributing to multi-agency meetings, case conferences and reviews
• Reporting progress to relevant multidisciplinary partners and escalating concerns when required
• Managing risk appropriately when supporting individuals experiencing crisis or challenging behaviour
• Working responsibly within relevant information and data-sharing frameworks
• Identifying creative and practical interventions that may prevent crisis or disengagement
• Working collaboratively with colleagues to agree priorities, share workloads and achieve team objectives
• Using reflective practice to contribute to continuous improvement in service delivery
Service Development
You will also contribute to the continued development of the Complex Lives approach by:
• Sharing learning and insight gained through frontline support
• Contributing to the development of preventative approaches for individuals with complex needs
• Supporting improvements to services and systems for vulnerable individuals
• Identifying opportunities to improve multi-agency working and service delivery
Health & Safety
You will be responsible for:
• Maintaining appropriate health and safety standards within the service
• Following organisational health and safety policies and risk management procedures
• Supporting the overall safety and security of the service
• Using appropriate security systems and procedures to manage identified risks
• Completing required wellbeing, health and safety checks
Administration & Record Keeping
The role will also involve:
• Maintaining accurate and confidential client records
• Recording relevant financial transactions
• Completing internal and external reports
• Recording and managing incidents and accidents appropriately
• Maintaining accurate documentation relating to case management and support
• Following relevant IT and cyber security policies and procedures
Essential Criteria
To be considered for this position, applicants must have:
• A minimum Level 3 qualification in a social care-related field, such as A-Levels, a Diploma in Health & Social Care or equivalent
• At least 2 years of paid experience working with vulnerable groups
• A full valid driving licence
• Full access to a car insured for business purposes
• A willingness and ability to undertake regular travel throughout the local area
